Designed to help organizations case manage Healthy Start program and services, WFDS supports the documentation and data collection that enables Healthy Start grantees meet the goals to reduce disparities in infant mortality and adverse perinatal outcomes.WFDS uses a wholistic platform to support HS Directors in achieving the many program goals that are required during the grant period

WFDS-CIR will provide a coordinated process for identifying clients in intake types (Preconception, prenatal, inter-conception, postpartum, male) that are eligible for services in a one or more of a variety of programs based on Home Visiting Referral Decision Tree and program availability.
